Patients are required to register for every service provided by Ward Memorial Hospital prior to receiving service. Admission / Registration hours are Monday - Friday, 7:00 am until 11:00 pm. Saturday and Sunday, 10:00 am until 4:00 pm. All information obtained during your registration and / or admission will be kept confidential. The Admissions staff has been trained and is familiar with the new Federally Mandated HIPAA guidelines. Every effort is made to protect patients' privacy while maintaining efficiency. When you come to register the following is a list of things you will need to bring each time you receive service from Ward Memorial:
You will be asked to sign appropriate consent and release forms that authorize hospital personnel to provide services that your physician has ordered.
In-Patient / Observation / Same-Day Surgery Visitation hours are from 2:00 PM to 4:00 PM and from 7:00 PM to 8:30 PM and no children under the age of 12 are permitted in patient care areas.
For your convenience the following forms have been provided
for you to print out and bring with you at the time of registration. Many
of these forms may require additional information from hospital staff.
