Patients are required to register for every service provided by Ward Memorial Hospital prior to receiving service.  Admission / Registration hours are Monday - Friday, 7:00 am until 11:00 pm.  Saturday and Sunday, 10:00 am until 4:00 pm.  All information obtained during your registration and / or admission will be kept confidential.  The Admissions staff has been trained and is familiar with the new Federally Mandated HIPAA guidelines.  Every effort is made to protect patients' privacy while maintaining efficiency.  When you come to register the following is a list of things you will need to bring each time you receive service from Ward Memorial:

  • Social Security Card

  • All Active Insurance Cards

  • Picture ID

  • Any Paperwork Given to You by your Physician

  • Living Will, If Applicable

  • Advanced Directives, If Applicable

  • Powers of Attorney, If Applicable

  • Any Medications from Home, If Applicable

You will be asked to sign appropriate consent and release forms that authorize hospital personnel to provide services that your physician has ordered.

In-Patient / Observation / Same-Day Surgery
To make your stay more comfortable, you may bring your own pajamas or nightgown, bathrobe, slippers, toothbrush, and other personal effects.  Please leave all of your valuables at home and do not bring cell phones into the hospital.  While you remain at Ward Memorial Hospital, you will be required to wear an identification band.  This helps ensure that you will receive the proper treatments and medications.  Please do not remove the band for any reason.

Visitation hours are from 2:00 PM to 4:00 PM and from 7:00 PM to 8:30 PM and no children under the age of 12 are permitted in patient care areas.

For your convenience the following forms have been provided for you to print out and bring with you at the time of registration.  Many of these forms may require additional information from hospital staff.
