Ward Memorial believes that the family and the home are an integral part of wellness.  That's why Ward Memorial Home Health was created, making it possible to bring the healing care and services of the Ward Memorial Hospital Team directly to you in your own home.  Our Team Members provide complete hospital based health programs for your recovery.  You receive professional medical personnel and services at a fraction of the cost of acute and skilled nursing facilities.  Home Health Care is your best choice for keeping the family together, lowering medical costs, and producing a positive environment for the patient who is recovering in the home.

Members of the Ward Memorial Home Health Care Team are experienced professionals in their fields and include Registered Nurses, Licensed Vocational Nurses, Licensed Physical Therapist and Assistants, Certified Nurse Aides, Home Health Aides, Registered Dietician, and Speech Therapist.

To Receive Home Health Care - Simply call our office or have your physician contact us.  One of our registered nurses will review your needs to determine if you meet the necessary requirements which are as follows:

  1. Patient has an acute illness.

  2. Patient is home bound.

  3. Patient is under the care of a physician.

  4. Patient needs part-time or intermittent skilled services.

  5. A reasonable expectation exists that the home health agency can meet the patient's medical, nursing, and social needs in the patient's home.

  6. Patient's home is a safe and adequate environment for care.

Payment Options - Ward Memorial Home Health accepts Medicare, Medicaid, Self-Pay, Private Insurance, and Charity.  For your convenience, we also process all  billing.


Availability of Services - Services are available Monday through Friday, 8:00 am to 5:00 pm.  The office is open to the community for free Blood Pressure checks, Blood Sugar checks, or to answer any questions.  On-call nurses are available 24 hours a day for patients who need additional care for urgent needs. 

Special Services - Ward Memorial Home Health offers the following special services:

  • Nutritional Support Counseling

  • Infusion Therapy

  • Medical Response Alert System

Referrals - Referrals for home health care may be made by you or your family, a friend, your physician, or any member of the community.   However, referrals by anyone other than your physician must have the approval of your physician.
